Iowa DOT Request for Driver Re-Examination

Physicians and Law Enforcement may use this form to request a Driver have a Re-Examination of their driving skills.
This form must be signed and emailed to

Iowa DOT Driver Fitness Dept.

The process does take some time. Each case will be investigated. At the time of this submission, you may begin the referral process with us for a Clinical and/or Driver Evaluation.
